To become one of our vendors, please complete the Vendor Agreement found below and email it to firstname.lastname@example.org along with the following items:
1) A copy of your license.
2) Proof of liability insurance and workman's comp insurance with Hawk Mgmt, Inc. listed as additionally insured. Please use our mailing address below.
4) A copy of your EPA certification.
5) W-9, if you are not a corporation.
6) Your contact information.
7) A voided check which includes routing and bank account information.
8) 3 references.
9) What areas do you service?
10) Do you work on an hourly basis or per project?
